Does the student have a parent or a sibling who has a CERTIFICATE OF ELIGIBILITY? If so:
Provide:
the child’s long form Birth Certificate, showing parents’ names
parent’s valid proof of Canadian citizenship. If it is your child’s sibling who has eligibility, then we need the citizenship of the parent in common (child and eligible sibling)
the parent’s or sibling’s Certificate of Eligibility (or the Quebec permanent code)
Did one Parent attend English Elementary and/or High School in Canada? If so:
Provide
child’s long Birth Certificate showing parents’ names
proof of Canadian Citizenship for the parent educated in Canada
a letter of attestation from the school or school board attended (on official school letterhead) to prove that the majority of youth education (elementary and/or high school) was 100% in English
Is the child attending, or did the child attend, the majority of Elementary or High School in English in Canada? (Excluding Private schools in Quebec and children who were granted a Temporary Stay Certificate of Eligibility - Bill 104). If so:
Provide
child’s long Birth Certificate showing parents’ names
parent’s proof of Canadian Citizenship
a letter of attestation from the school or school board attended (on official school letterhead) to prove that the majority of youth education (elementary and/or high school) was 100% in English
Does the child have a sibling who is attending, or has attended, the majority of English elementary or secondary school in Canada? (Excluding Private schools in Quebec and children who were granted a Temporary Stay Certificate of Eligibility- Bill 104). If so:
Provide
a letter of attestation from the school or school board attended (on official school letterhead) to prove that the majority of primary education was 100% in English
parent’s proof of Canadian Citizenship
child’s and sibling(s)’ long Birth Certificate showing parents’ names
Did a grandparent receive the majority of their education in English in Canada If so:
Provide
child’s and parent’s long Birth Certificate showing parents’ names
grandparent’s Birth Certificate
a letter of attestation from the school or school board attended (on official school letterhead) to prove that the majority of youth education (elementary and/or high school) was 100% in English
proof that the parent studied in Quebec (report card or transcript)
Are the parents residing in Quebec temporarily* on a STUDENT or WORK AUTHORIZATION? If so:
Provide
parent’s VALID Work or Student Authorization.
child’s long Birth Certificate showing parents’ names (must be in English or French) and Immigration papers linking the whole family.
Is the family here on Diplomatic status? If so:
Provide
parent’s letter of protocol and letter of attestation for child/children from Protocol Quebec
child’s long Birth Certificate showing parents’ names (must be in English or French)
